Abstract

Social work and discharge planning in the hospital setting generally implies working in collaboration with an interdisciplinary team of doctors, nurses, physical therapists, nutricionists, utilization review case managers, as well as with other community support systems outside the hospital (nursing homes, rehab centers, home care agencies, medical supplies agencies, etc.). In these units of the hospital, the social worker prepares patients for their discharge once their medical condition has been stabilized and the patients can return to their home or to a health care facility if so recommended by the interdisciplinary team. Some patients might be in need of home care services or medical equipment (wheelchairs, walker, oxygen, etc.).
